Open Entry: Entry Page View – Form Fields & Design

Sweepstakes Form Fields

OneID automatically captures the following information:

  • First Name
  • Last Name
  • Address
  • Address 2
  • Address 3
  • City
  • State
  • Zip Code
  • Phone Number
  • Email

To add more fields, click on the drop down 

Choose the type of response option for acquiring information and fill out the question and predetermined answer choices if necessary. Each option will ask if the question is required and allow space to write a custom error message.

Checkbox (Group)

In the checkbox (group), you can have multiple checkboxes, add a write-option box, create a max for boxes checked, and choose to have a vertical or horizontal display of boxes.

Checkbox (Single)

In checkbox (single), you can add another requirement, have a URL, or have it checked off by default.

Dropdown List

In the dropdown list, you can add multiple options to choose from. 

Radio Buttons

In radio buttons, you can add multiple options for viewers to choose from as well as display the options vertically or horizontally.

Text Area

In text area, you can set a max of characters, configure height in rows, and width in percentages. 

Text Input

In text input, a max of characters can be set, but the text box size will remain the same.

When more question and response fields are added the fields and official rules checkbox can be moved up and down by clicking on the up and down arrows.

Official Rules & Terms of Use: Edit the Official Rules & Terms of Use copy and error message by clicking on the . Markdown syntax is needed to add in the two links.

Enter Now: The Enter Now button can be customized in button color combinations and text.

Official Rules: Official Rules can be uploaded via URL, PDF, or Text Composer on the Campaign Page or “Manage” tab. Please see Campaign Settings for visuals. Please note the Official Rules will remain the same for all snapshots in a given campaign. 

Add Follow Links: Add links for Facebook, Twitter, Instagram, or company newsletter to allow viewers to follow. Text, font, and colors can all be customized.

Note: Follow links must include HTTPS in the URL (Sample: https://telescope.tv/)

Colors and Design

Make each sweeps unique by changing the background and text options or adding a background image.

Backgrounds, text colors, sizing, fonts, and url colors can all be customized in the sweeps template.
